Tips for Success:​

1) Make appointments for these exams over the summer.  Many pediatric and family practice physician offices book quickly over the summer with anticipation of the October 15th deadline mandated by the State of Illinois.  Waiting until September to schedule examinations may result in an appointment date after the October 15th deadline.  Don't delay, make your appointment today!

2) Your child's physician will know which immunizations are necessary for their age/grade level to be compliant with the State of Illinois school health requirements.  You can print the Minimum Health Requirements information page that is linked above if you have questions for your child's physician about specific immunizations. 

3) Print the above linked exam forms and complete your child's personal information in advance as appointment reminders.  Be sure to take the forms with you to the appointment, or request a printed copy at the medical provider's office for submission to the school. 

4) A parent should complete the "Health History" section on page 2 of the State of IL Certificate of Child Health Examination form linked above.
